Tuition is due on the 1st of each month. Tuition is based on classes of instruction, not days danced per week or holidays involved. You may elect in enrolling on automatic pay. If so, please provide credit card and signature below. You will be billed on the 1st of each month. You may cancel automatic deductions from your account at any time with proper written notice (received 30 days prior to payment date). You may withdraw from classes at any time with proper written notice (received 30 days prior to payment date). Annual registration fee ($70 for NEW students and $55 for RETURNING students), late fee $35 (after the 1st of each month). Performance/costume fees, master classes, auditions, private classes, etc. are billed separately and due upon receipt.

I will be contacted first in the event of a medical emergency. If I cannot be reached, my emergency contact person will be notified. If my emergency contact person or I cannot be reached, I hereby authorize Classical Ballet of Miami School or its appointed representatives to sign for medical care. I hereby give permission for my child to be treated by an emergency medical technician or other medical professional as deemed necessary by Classical Ballet of Miami School. In the event of a non-emergency medical situation, Classical Ballet of Miami School may recommend a medical professional for care. The decision of whether to consult a medical professional for non-emergency care will be my decision.
I understand that Classical Ballet of Miami School is not responsible for any injuries sustained at any time and/or at any location while participating in classes/events and/or traveling to or from classes/events. I recognize that my child’s participation may expose him/her to the risk of injury. I accept this risk and hereby release Classical Ballet of Miami School, its agents, employees, and faculty from all liability for personal injury, illness, or property damage occurring during instruction or performance, whether or not caused by the negligence of Classical Ballet of Miami School, its agents, employees, and/or faculty. I understand that any studio, gymnasium, auditorium, or theater is not accountable for any injury, illness, or property damage occurring during performance or instruction. I certify that my child is in good health and capable of participating in all of the activities and classes. I understand that dance instruction involves kinetic corrections to the body that may involve physically touching the student as a part of regular class work and rehearsals.
We believe that our students, staff, and school benefit from positive recognition and there may be occasional media coverage that involves our students. We ask permission to release pictures and/or other communication to the media on occasion. This may include but is not limited to newspapers, magazines, news station coverage, newsletters, and our website. By your signature, hereto you authorize Classical Ballet of Miami School to use your child’s likeness. The Classical Ballet of Miami School has my permission to take photos, videos, and/or films of me, my son or daughter and consent to use such materials for promotional purposes by Classical Ballet of Miami School.
It may occasionally be necessary for Classical Ballet of Miami School to cancel scheduled classes due to inclement weather, teacher illness/inability to find an adequate substitute, and/or performance schedule conflicts. We will give notice of any cancellations as far in advance as possible. Cancelled classes may be rescheduled if another class of the same level/type is not available. If the same class level is available on another day, students should make up the missed class in their current level or one level below. They may make-up classes at a higher level ONLY at the discretion and approval of their teacher or director. Students should make-up missed classes in a timely fashion (WITHIN THE MONTH). Tuition will not be pro-rated for student absences. Tuition is non-refundable, no exceptions.
IT IS IMPORTANT FOR STUDENTS TO MAINTAIN GOOD ATTENDANCE IN THEIR CLASS AS TOO MANY ABSENCES MAY AFFECT A STUDENT’S PROGRESS AND FUTURE CLASS PLACEMENTS.
Once a student is enrolled, tuition is due by the first of each month (A $35.00 late fee will be applied to my account after the close of business on the 1st) and is non-refundable, no exceptions. Bounced Checks charge: $30.00
Students wishing to make any schedule changes must do so by notifying Classical Ballet of Miami School either by phone or email. This is to ensure proper tuition disbursement. Students completely withdrawing from Classical Ballet of Miami School must also fill out a schedule change form; accounts will continue to accrue monthly tuition until proper written notice of withdrawal has been received by the office. Students that have withdrawn from Classical Ballet of Miami School without turning in the necessary paperwork and paying owed tuition may also be billed and/or turned over to a collections agency.
